In consideration of Dao Speaks Public Speaking Club (the "Club") allowing my child(ren) (the "Participant") to participate in public speaking, leadership, and educational activities (the "Activities") sponsored, hosted by, or otherwise related to the Club, I agree as follows:
- Authority. I am the parent or legal guardian of the Participant and have the legal authority to enter into this Agreement. I represent that: a) I have authority to enter into this Agreement on behalf of anyone else who has legal rights regarding the Participant; or b) everyone else with legal rights regarding the Participant has signed this release.
- Voluntary Participation. I agree that the Participant's involvement in the Club's Activities is voluntary and undertaken for educational and personal development purposes.
- Publicity Release. I grant the Club and its parent organization, Toastmasters International, permission to record, use, reproduce, and publicly display (including on the Club's website, social media, newsletters, and promotional materials) the Participant's name, likeness, voice, and performance materials (e.g., speeches, presentations) for the purpose of promoting the Club's mission, reporting on Club events, and supporting the educational objectives of the program.
- No Duty to Act on Conditions Specific to Participant. I understand and agree that the Club and its volunteers are not medical professionals and are not qualified to provide medical or psychological evaluation or treatment. Therefore, I understand and agree that the Club has no duty to utilize the information provided regarding medical, emotional, or other limitations faced by the Participant, beyond making reasonable accommodations for participation in public speaking activities.
- Authorization to Engage Medical Treatment. I grant permission for the Club, through its designated officers or volunteers, to authorize medical treatment for the Participant, to call 911 (or other appropriate emergency services) for emergency medical aid, or take other reasonable measures to secure medical treatment if, in the Club's sole judgment, the Participant becomes ill, sustains an injury, or otherwise requires medical treatment during a Club event. I give consent to any physician, emergency aid responder, or other health care provider to administer drugs or medicine or to perform such medical treatment as such person determines necessary for the relief of pain or to preserve the Participant's life or health. I assume full responsibility for all medical, rescue, transportation, and other expenses incurred on behalf of the Participant and will fully and immediately reimburse the Club for any of these expenses that the Club, in its sole discretion, chooses to advance.
- Release of Claims. I, on behalf of myself, the Participant, and our heirs, executors, and assigns, hereby release, waive, and forever discharge the Club, Toastmasters International, and their officers, directors, employees, agents, and volunteers (collectively, "Released Parties"), from any and all claims, demands, liabilities, and causes of action of any nature, known or unknown, fixed or contingent, which may arise from or are in any way related to the Participant's or my own participation in the Activities, including travel to and from the Activities. This release includes, but is not limited to, claims based on the negligence of the Released Parties.
- Club Communication & Conduct. To facilitate communication, the Club may provide access to platforms such as email lists, messaging groups, or online forums (collectively, "Club Platforms"). I agree that I will not use these Club Platforms to post or disseminate advertisements, promotional content, or solicitations for my own business or commercial activities. I further agree to abide by the Club's code of conduct and all applicable terms of use for these platforms, fostering a respectful and supportive environment for all members.
